MESH - patient documents - rejecting on the recipient-side

Hello,

GP practices receive patient documents on systems like EMIS Web via the NHS MESH transport mechanism. When reviewing the document, users have the option to “reject” the document, which returns it back to the sender. I wanted to get more information on this, its use case and whether this is a reliable way to let the sending organisation know a document can’t be processed because of a business reason (e.g. patient no longer registered at the GP practice).

Could someone provide some information? From my understanding this is only available for the newer FHIR standard workflow IDs (e.g TOC_FHIR_EC_DISCH, TOC_FHIR_IP_DISCH, etc) used to send patient documents to GP practices. However my concern is that the sending system may not have built out proper handling for rejection cases from the GP practice. It could lead to the following scenario:

  • The sender sends a document to the GP practice, thinking the GP practice has been notified.
  • The GP practice would send a rejection thinking the sender has been notified.
  • The affected patient gets stuck in limbo.

Hoping someone here can advise.

Kind regards,
Faraz
