What Clinical Checks Are Considered Most Critical or Sensitive by the NHS for Community or Clinical Pharmacy Systems?

Hi all,

I’m exploring the implementation of a clinical decision support system that performs up to 40 clinical checks—similar to what’s used in solutions like Titan PMR.

I’d like to gather feedback from the NHS community:

Which clinical checks are considered most critical or high-risk by the NHS or regulators (e.g., MHRA, NICE, NHS Digital)?

Which areas trigger the most safety alerts or interventions in NHS practice?
Examples I’m considering:

  • Drug–drug interactions
  • Age/weight-based dose errors
  • High-risk meds (e.g., anticoagulants, valproates, controlled drugs)
  • Allergy/intolerance mismatch
  • Duplicate therapies

Are there others that the NHS is particularly sensitive about? Or any guidance documents you’d recommend reviewing (e.g., DCB0129, ISMP, NHS Clinical Safety Standards)?

Any insight or real-world examples would be much appreciated!
