I am currently looking for a sample or collection of sample/dummy/anonymised various NHS Patient letters.
The letters should not contain real patient information. The structure and format of the letters is essential.
Where can I find example documents such as Discharge summary, Appointment Letter, Diabetic Patient letter etc?

There is set information which must be included in clinical transfers of care such as discharge.
